How To Order

1. Print out Printable Order Form in Landscape orientation.

2. Fill out form as completely and as neatly as possible.

3. Your satisfaction is my #1 goal, so please include your e-mail address and/or phone number so I have a way to reach you in case any questions arise in processing your order. (i.e. spelling verification, thread color, etc.)

4. Be sure to add the Shipping & Handling charge to your order. For Ohio residents, first add 6.25% to your merchandise total, then add Shipping and Handling. In other words, sales tax does not apply to Shipping and Handling.

5. Mail your completed order form along with check or money order made payable to Leslie Malick to: Pepperhill Design Co.

6. For those who include their e-mail address on the order form, I will send you a confirmation when I receive your order and when your order is shipped.

7. All orders will be shipped USPS Priority Mail (2-3 days Delivery).

8. All personalized items cannot be returned unless there is an error made by the seller. In that case, the item must be returned at the buyer's expense, and then the item will be replaced with Shipping and Handling paid for by the seller. No exceptions!
